Creating an Instructor Agreement PDF Instructions
Students requesting additional flexibility beyond certain outlined accommodations are required to schedule a time to meet with their instructors to complete the Instructor Agreement form for any applicable class.
The purpose of the Instructor Agreement form is to facilitate communication between students and faculty to establish conditions of the accommodation that provide reasonable flexibility based on the student’s disability needs and the requirements of the course.
The instructor agreement is only required in situations when the standard accommodation description does not apply. Please consult the disability notification letter for details and the disability specialist if you have questions.
